ABSTRACT
Effective management of stroke patients is essential to prevent physical and mental disabilities. This study aims to implement nursing care management for non-hemorrhagic stroke patients using a case study approach at Arifin Achmad General Hospital in Riau, with an eight-day observation period in June 2024.The nursing care management approach encompasses planning, organizing, implementing, and evaluating based on prioritized nursing issues. Interventions follow the Indonesian Nursing Diagnosis Standard (SDKI), focusing on three primary areas. First, physical mobility impairment was addressed through passive Range of Motion (ROM) exercises to improve muscle strength and joint flexibility. Second, verbal communication impairment was managed using hand signals as an alternative communication method, along with teaching the patient to speak slowly. Third, fall risk was mitigated through education on risk factors and preventive measures for both the patient and family. Results over the eight-day nursing period indicated significant improvement. The patient's Glasgow Coma Scale (GCS) score reached 15, indicating full consciousness. The patient independently performed passive ROM exercises with increased muscle strength rated at 4+/5. Morse Fall Scale (MFS) assessment showed a low fall risk following family education. Additionally, hand signals and slower verbal communication were successfully adopted, enhancing interaction. This structured approach demonstrates that tailored, evidence-based interventions effectively support recovery and reduce complications in non-hemorrhagic stroke patients.
